(RN) Oncology Clinic RN - Transitions of Care - 130667

Job Description

DESCRIPTION

The Oncology Transitions of Care RN provides exceptional clinical care utilizing the nursing process (assessment, planning, intervention, evaluation, patient/family education) in triage and care coordination within multiple Oncology Transitions of Care initiatives. This initiatives, focused on preventing unplanned hospitalizations and ED visits, including discharge coordination for all established oncology patients discharged from ED or hospital within 24-48 hours, proactive outreach to new infusion patients, and triage of patients for care by the Same Day Cancer Clinic services team or primary oncology team. In addition, this position will have the opportunity to work within Same Day Cancer Care Clinic services to provide same day oncology care to patients across all disease sites and modalities, ensuring expert triage and assessment (in person and telephone) of urgent symptoms or issues. This position will have expert written and verbal communication skills with patients, families, faculty and all health team members across multiple departments, ensuring efficient and excellent patient care in alignment with appropriate clinical algorithms and pathways.

MINIMUM QUALIFICATIONS
  • Must be a graduate from an accredited school of nursing.

  • Registered Nurse (RN) license issued by the State of California.

  • BART or BLS at time of hire with commitment to get BART within six (6) months of hire date.

  • Must have a minimum of three (3) years of recent demonstrated experience in outpatient or inpatient Oncology, Emergency Department, or Urgent Care.

  • Must be able to function in a fast-paced environment, have excellent interpersonal and communication skills, and consistently demonstrate empathy and kindness to all staff and patients.

PREFERRED QUALIFICATIONS
  • BSN or MSN.

  • Oncology Certification (OCN).

  • Two (2+) or more years of outpatient oncology experience.

  • Experience with vulnerable patient populations.

  • Telephone triage experience.

  • Multi-disciplinary care coordination experience.

  • Patient and family education experience.

SPECIAL CONDITIONS
  • Must be able to work various hours and locations based on business needs.
  • Employment is subject to a criminal background check and pre-employment physical
  • Selected candidate may be hired at the CNIII or CNIV level pending review and approval by the Nurse Executive Council.

Pay Transparency Act

Annual Full Pay Range: $123,902 - $173,659 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $59.34 - $83.17

Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
