Patient Portal Representative, LVN

The Patient Portal Representative, LVN will be responsible for the facilitation and/or resolution of electronic patient portal requests to meet the needs of patients. Responds to patient requests to provide information, follow-up, or effectively coordinate with other members of the care team.

Responsibilities:

  • Responds to a variety of patient requests from the patient portal to provide resolution within company's established guidelines and timeframes.
  • Assesses patient needs to efficiently and effectively determine the best course of action to ensure positive patient outcomes, which may include collaboration with both internal and external patient care teams.
  • Utilizes clear and concise electronic written and verbal communication skills to draft correspondence, complete documentation, and respond to others.
  • Uses company and departmental protocols and procedures to resolve patients' needs.
  • Coordinates services with other departments for follow-up.
  • Uses problem-solving skills to determine the best course of action which will be satisfactory to the patient and organization.
  • Maintains up-to-date knowledge and skills in professional, clinical, and organizations technology areas.
  • Uses critical thinking and communication skills to assess and process patient portal requests based on evidence-based guidelines.
  • Consistent with internal policies and procedures, accurately documents their work.
  • Performs other duties as assigned.

Requirements:

  • Graduation from a recognized practical nurse program or equivalent.
  • High School Graduate (or GED equivalent).
  • Minimum of two (2) years of experience in acute care nursing or outpatient clinic
  • Current Basic Life Support (BLS) and Cardiopulmonary resuscitation (CPR)
  • California State Board LVN License required.
  • Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Licensed for a minimum of 3 years; No more than 2 violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving with 7 years

In the spirit of pay transparency, we are excited to share the base range for this position, exclusive of fringe benefits or potential bonuses.

$30.00 - $36.65

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
