Family Health Centers of San Diego (FHCSD) is looking for a Coordinator, PACE Transportation - The PACE Transportation Coordinator responsible for coordinating and scheduling transportation assignments of PACE attendees. Ensure communication is clear and drivers follow through as directed. Organize routes and schedule daily transportation for PACE attendees to and from the PACE Center.

Responsibilities:

• Schedule transportation for all PACE participants. Respond appropriately to needs as identified by the interdisciplinary Care Team.
• Coordinate transportation for PACE participants to ensure that the transportation is on schedule, timelines are met, and are at the highest level of operational efficiency.
• Coordinate schedules, cancel schedules, and reschedule trips as needed. Make adjustments to meet unanticipated changes and communicate those changes to the appropriate affected personnel.
• Model and promote service excellence by developing and maintaining positive respectful relationships with PACE participants, families/caregivers, team members, and all staff throughout the organization.
• Maintain all records for transportation, and ensure established policies and procedures are communicated and followed.
• Perform other duties as assigned.

Requirements:

• High School Diploma/GED required.
• Meet organizational driving requirements.

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
